Pulmonary Blood Volume in Congestive Heart Failure
نویسنده
چکیده
IN ANALYZING the clinical and hemodynamic features of congestive heart failure due to myocardial disease or associated with mitral stenosis, many authors indicate or clearly imply that a significant augmentation in pulmonary blood volume plays a major role in the pathogenesis of pulmonary congestion and edema.'-6 These conclusions have depended largely upon indirect evidence such as clinical evaluation, the interpretation of chest roentgenograms, and the measurement of pulmonary vascular pressures and central blood volume. Utilization of indicator-dilution techniques, combined with simultaneous right and transseptal left heart catheterization, has made more precise quantitation of the true pulmonary blood volume (PBV) possible. The technique of Milnor and associates7 has been employed in our laboratory in the study of several hundred patients with valvular and other forms of acquired heart disease. It is the purpose of this presentation to (1) report the changes in PBV in a group of patients with heart failure associated with severe mitral stenosis and in another group of patients with nonvalvular congestive heart failure; (2) compare values of PBV in these patients with those obtained in a separate group of patients who had normal hemody-

Reversible Pulmonary Hypertension in an Infant Treated with Diazoxide
Background: Diazoxide is the main therapeutic agent for congenital hyperinsulinism. The drug is generally well tolerated; however, in this report severe adverse effects including heart failure (HF) and pulmonary hypertension (PH) in an infant are reported.Case report: A sixteen-day male infant with persistent hypoglycemia and with diagnosis of congenital hyperinsulinism underwent near total pan...
متن کاملVein of Galen Aneurysmal Malformation and High-output Cardiac Failure in a Newborn
Background: Vein of Galen aneurysm (VGA) is the most common form of symptomatic cerebrovascular malformation in neonates. It develops in a fetus in the first trimester of pregnancy due to unknown reasons, but it is likely to have a genetic etiology. The prognosis of VGA is usually poor, particularly in newborns with heart failure due to high-flow intracerebral shunt. Surgery and endovascular em...
متن کاملPrecipitating factors of congestive heart failure in admitted patients of Fatemieh Hospital in Semnan, Iran
Introduction: Congestive Heart Failure (CHF) is a pathophysiologic state in which an abnormality of cardiac function is responsible for the failure of the heart to pump blood at a commensurate rate with the requirements of the metabolizing tissues and/or allows it to do so only from an abnormally elevated diastolic volume. In this study, the prevalence of precipitating factors of CHF in Semna...
متن کاملThe effects of hexamethonium on certain manifestations of congestive heart failure.
Hypotensive doses of hexamethonium were administered intravenously to 19 patients with various types of heart disease in congestive failure. Clinical improvement as judged by the usual methods was seen in most cases. Intracardiac or pulmonary artery pressures paralleled the fall of arterial pressures following hexamethonium in four patients with hypertensive heart disease in congestive failure....
متن کاملFluid Volume Overload and Congestion in Heart Failure: Time to Reconsider Pathophysiology and How Volume Is Assessed.
Volume regulation, assessment, and management remain basic issues in patients with heart failure. The discussion presented here is directed at opening a reassessment of the pathophysiology of congestion in congestive heart failure and the methods by which we determine volume overload status. Peer-reviewed historical and contemporary literatures are reviewed. Volume overload and fluid congestion...
